for visascreen purpose, no need for ielts/toefl if you're a green card holder or a US citizen. visascreen is an immigration requirement. green card holders or US citizens are not required to get a visascreen certificate.
for state licensure requirement, ielts/toefl may or may not be needed depending on the state. in the case of the one asking the question here, her chosen state is CA which does not require ielts/toefl. in some states such as MD, AZ, IL, etc... proof of english proficiency is required.
