hi topaz,i dont remember the exact location but im sure u have go to the website of www.cgfns.com click about application. there are forms with the heading LICENSE VERIFICATION like what we used sa PRC for our license(did you get yours?) fill it up and sign. go to PRC and as usual pay. i wont pay for FEDEX mahal pa, just the regular i think 125 pesos but i wont be sending the original DOLE cert as much as possible, baka photocopy lng.update kita for whatever step ill be making, share your thoughts too....gudluck!
