that is a big NO. you don't include your TOR with your application forms. Basta remember lahat ng proofs that required authenticity must come from the governing office. so yung TOR mo dapat school mo din magsent. If you were to send your own TOR make sure you send it separately and it has the same postal code with your school. e.g. kung qc school mo qc din dapat ang post office mo para safe.
